At Home With Bridge to Care

We strive for independence, balance, and peace. This is our goal for the residents of Bridge to Care. We know there are more people in need of housing than space available, and we are here to change that. Bridge to care is a Community Integrated Living Arrangement for adults with disabilities and special needs. Nestled in the heart of Lake County, Illinois, our CILA home is dedicated to providing exceptional care for residents who require some assistance and guidance to live an independent, balanced, and peaceful life.
We understand the unique needs of our residents, and our mission is to offer a home-like environment that goes beyond traditional CILA’s. Founded by Desiree Cunningham, an experienced family nurse practitioner (FNP), Kiarra Dacres, a licensed community association manager (IL-CAM), and Ruth Kenny a licensed registered nurse (RN); our facility combines medical expertise with a focus on well-being and housing needs.
With eight beds featuring private and semi-private rooms, our intimate setting ensures personalized attention and fosters a sense of community. Unlike larger facilities, our residents experience a mentally stimulating environment, surrounded by fellow residents, dedicated staff, and engaging activities. We prioritize quality care tailored to individual needs, recognizing the importance of maintaining dignity and independence of all residents.
Our services range from daily activities to medication management, community involvement, and resident assistance; all overseen by licensed professionals. We are committed to the safety and well-being of our residents, taking all measures including bathroom grab bars, and offering a range of amenities, including home-cooked, meals, social activities, and 24-hour staffing.
