Bridge to Care offers a full continuum of care, providing unique benefits to individuals with Intellectual and Developmental Disabilities (IDD). Our services ensure smooth transitions to appropriate settings based on changing preferences, abilities, needs, age, and levels of disability. We maximize training and employment opportunities and address the growing need for services as individuals age, allowing us to serve medically complex individuals with IDD.
Our mission is not to take your family member from you but to share in the care and support of your loved one. We see this as a shared responsibility, which includes regular home visits, medical appointments, and various meetings. Families are integral to Bridge to Care, becoming part of our extended family when their loved one joins us. The high-quality care and programs we offer are made possible by the commitment and involvement of parents, families, guardians, and friends.
Bridge to Care serves adults with Intellectual and Developmental Disabilities, offering programs for both ambulatory and non-ambulatory individuals with a range of intellectual and developmental disabilities, from profound to moderate levels. Individuals admitted must be from the State of Illinois, preferably living in the Chicago Metropolitan area and suburbs. Funding for all our programs is provided by the Illinois Department of Health and Family Services (HFS) and Social Security.
All adults participate in Bridge to Care’s vocational programs, employment services, and day programs when available. Some have jobs in the community or within our home. Admission applicants are considered "on-file" only. Before being placed on the waiting list, applicants and their families undergo an interview and generally have an overnight observation in our home. Arrangements for observations depend on bed availability. After the observation, a determination is made regarding acceptance to the waiting list.
